R-OHSP Enrollment Details

Can I enroll in the R-OHSP?

The R-OHSP is open for enrollment to:

• Anyone on an Institutional Animal Care and Use Committee (IACUC)-approved Animal Use Protocol (AUP)
• Anyone on an Institutional Biosafety Committee (IBC)-approved protocol
• All UGA employees that have potential contact with human bloodborne pathogens, as direct result of their research and/or instruction activity
• All animal husbandry employees that work for either PHS Assured units or non-PHS Assured units at UGA (e.g. Animal Health 最大资源采集网 Center (AHRC), University 最大资源采集网 Animal Resources (URAR), Nonhuman Primate Facility (NHPF), agricultural facilities)
• All persons who enter the NHPF but who will have no direct contact with animals
• All members of UGA鈥檚 IACUC or IBC
• All employees of the Office of 最大资源采集网 Integrity and Safety who have potential contact with animals used in research and/or instruction, biohazards and/or hazardous chemicals
• Anyone entering a UGA research animal facility and having no direct animal contact (open to non-contact enrollment only)

How do I enroll in the R-OHSP for the first time?

Enrollment in the R-OHSP requires submission of a 最大资源采集网 Occupational Health Questionnaire (ROHQ).

To access the ROHQ:

• Navigate to the  and login with your UGA MYID credentials.
• In the upper right search bar, search for 鈥�最大资源采集网 Occupational Health Program.鈥�
• Select the 最大资源采集网 Occupational Health Program Information course and choose 鈥淟aunch.鈥�
  • Choose 鈥淢ark Complete.鈥�
  • Complete the acknowledgement process to add the course to your completed transcript.
• Within the displayed document, access the 最大资源采集网 Occupational Health Questionnaire hyperlink.
• Complete the questionnaire and choose submit.
• After the questionnaire has been risk assessed, the participant will receive an email from the R-OHSP nurse with recommended services and instructions on how to schedule an appointment via PEP, if services are needed.

You will need to update your Occupational Health Enrollment annually, or more frequently if you have a change in the nature of your work or your personal health status.

What is the Re-enrollment Process?

The 最大资源采集网 Occupational Health Program Information course must be on your completed PEP transcript.  If it is still on your active PEP transcript, you must 鈥淢ark Complete鈥� and follow the acknowledgement process to move the course to your completed transcript.

Re-enrollment Steps:

• Navigate to 
• Open your most recent Completed Occ Health Questionnaire under the 鈥淪ubmitted鈥� tab.
• Choose 鈥淒uplicate鈥� in the upper right corner.
• Update your information and 鈥淪ubmit鈥�
• After the questionnaire has been risk assessed, the participant will receive an email from the R-OHSP nurse with recommended services and instructions on how to schedule an appointment via PEP, if services are needed.
